Admission Process

At BlueWave Living, staff respect people’s needs and choices and take an individualised approach to care with an emphasis on healthy, active and positive living.

The process of applying for admission to BlueWave Living is designed to be as simple as possible. Placement will depend on room availability and care needs. For any applicants who BlueWave Living is unable to immediately accommodate, the organisation happily keeps the applicants information on the accommodation waiting list.

To be eligible for admission to BlueWave Living applicants must:

  1. Have approval for permanent residential care from the Aged Care Assessment Team (ACAT)
  2. Have completed and/or submitted a Residential Aged Care Calculation of your Cost of Care (SA457) or Residential Aged Care Property Details for Centrelink and DVA Customers (SA485) to your pension provider

If you wish to apply for residential care at BlueWave Living you will need to provide a copy of the following documentation:

  1. An up to date Support Plan from the Aged Care Assessment Team (ACAT) or the approval code for Permanent Residential Care
  2. A completed admission application form. Please click here to download a copy of the BlueWave Living admission form
  3. Copies of Power of Attorney and Enduring Guardianship
  4. A copy of your assessment from Centrelink or DVA outlining your income and assets and residential aged care fees.

When a room becomes available applicants will be contacted. BlueWave Living provides different levels of care including dementia support and palliative care. Each applicant is assessed on their care needs as to what room would be suitable

ACAT Assessments

To be eligible for admission to BlueWave Living you must first be assessed by your local Aged Care Assessment Team (ACAT).

A referral to the Aged Care Assessment Team is usually made through your GP or local hospital Social Worker.

ACAT can help you decide which level of care you need. Assessments are a consultation between you and social workers who can then advise whether you are capable of living independently or require the support of an aged care facility such as BlueWave Living.

The Aged Care Assessment Team for the Central Coast can be contacted on 02 4304 0700.

Asset and Income Assessment

People entering aged care, and who have the financial capacity to do so, will be asked to contribute to the costs of their residential aged care. Residents seeking government assistance with their accommodation payments will need to have their income and assets assessed by Human Services or DVA.

The assessment of your assets and income will determine:

  • The amount, if any, of your Means Tested Care Fee
  • Whether you’ll receive assistance from the Government towards accommodation costs
